Building a Statewide Behavioral Health Referral Network
Behavioral health referrals were often managed through phone calls, faxes, spreadsheets, and disconnected organizational processes. HEALTHe Insights partnered with the Delaware Division of Substance Abuse and Mental Health to help shape the statewide strategy, translate provider needs into requirements, support system selection, coordinate implementation, and build the engagement and adoption structure required for statewide use.
The work established a shared referral infrastructure that improved visibility across organizations and created the foundation for Delaware’s later expansion into statewide behavioral health care coordination.
From Electronic Referrals to Statewide Behavioral Health Care Coordination
As Delaware moved beyond electronic referrals, providers and state leaders needed a broader way to coordinate care across behavioral health, physical health, crisis, and community services.
HEALTHe Insights served as a core strategy and customer-side integration partner for DTRN360, helping align program goals, provider workflows, technical requirements, vendor responsibilities, funding strategy, communications, and implementation across a complex statewide environment.
The result is a statewide platform and operating framework designed to support more coordinated communication among care teams while adapting as provider, program, data, and policy needs evolve.
Implementing the Delaware Psychiatric Center’s First Electronic Health Record
The Delaware Psychiatric Center needed to implement its first electronic health record while balancing clinical requirements, operational complexity, stakeholder readiness, vendor decisions, and significant resource constraints.
HEALTHe Insights supported workflow assessment, strategy, vendor evaluation and selection, implementation oversight, project coordination, stakeholder engagement, and change management.
The implementation created a new foundation for real-time documentation, stronger communication, more coordinated transitions of care, and improved operational and clinical reporting.
Connecting Behavioral Health and Social Care Through Closed-Loop Referrals
Behavioral health outcomes are often affected by needs such as housing, transportation, food access, and social support. Providers needed a practical way to identify those needs, connect people with community resources, and understand whether services were received.
HEALTHe Insights supported the strategy, workflow design, partner alignment, technology planning, communications, and implementation structure required to incorporate social care referrals into Delaware’s broader care coordination model.
The work extended the statewide coordination model beyond clinical services by helping participating organizations connect individuals to community resources through a more coordinated referral process.
