The Challenge
Electronic referrals improved access and visibility across Delaware’s behavioral health system, but providers and state leaders needed a broader view of each person’s journey across behavioral health, physical health, crisis, and community services.
Moving from referrals to statewide care coordination required more than new technology. It required a practical operating model, data governance, provider engagement, vendor coordination, communications, sustainable funding, and long-term adoption support.
HEALTHe Insights’ Role
HEALTHe Insights served as a core strategy and customer-side integration partner, helping DSAMH connect program goals, provider workflows, technical requirements, vendor responsibilities, funding strategy, communications, and implementation across a complex statewide environment.
Our Approach
- Helped design the strategic framework for an integrated statewide behavioral health care coordination platform.
- Supported system selection through formal requirements gathering, vendor evaluation, and assessment of statewide operational needs.
- Conducted more than 50 interviews and working sessions with hospitals, treatment providers, residential programs, outpatient clinics, peer organizations, state agencies, and community partners.
- Translated clinical and operational workflows into configuration, integration, onboarding, and training requirements.
- Coordinated across technology vendors and partners supporting EHR integration, ADT data, electronic referrals, data exchange, privacy requirements, and implementation milestones.
- Developed key Medicaid APD and funding strategy components to support federal alignment and long-term sustainability.
- Led communications and creative support, including project branding, website development, messaging, outreach materials, video concepts, provider communications, and adoption support.
The Outcome
DTRN360 established a statewide platform and operating framework for behavioral health and social care coordination. The initiative increased shared visibility, supported more coordinated communication among care teams, and created infrastructure that can continue to evolve as provider, program, data, and policy needs change.
